Final Forms - Athletic Forms to Complete Before Trying Out

Parents must register their student in Final Forms and complete all forms before participation is allowed. This needs to be done every school year.

Athletes must have a sports physical on file. Starting in the 2025–2026 school year, students only need one sports physical during middle school. If your child has a major health change—like surgery, a hospital stays, or a new long-term condition—they will need a new one.

 

Together with the Athletic Director, each Head Coach will host a parent meeting. The code of conduct will be reviewed. Additionally, information about concussions, hydration, opioid addiction, and cardiac concerns will be covered at the parent meeting.
