Final Forms - Athletic Forms to Complete Before Trying Out

Parents must register their student in Final Forms and complete all forms before participation is allowed. This needs to be done every school year.

Athletes must have a sports physical on file. Together with the Athletic Director, each Head Coach will host a parent meeting. The code of conduct will be reviewed. Additionally, information about concussions, hydration, opioid addiction, and cardiac concerns will be covered at the parent meeting.
